Tryout Authorization Form

RICHMOND, Ky. – The Eastern Kentucky University baseball program will hold walk-on tryouts on Sept. 8 at 4 p.m. 
 
The tryouts will be held at the baseball indoor facility next to Earle Combs Stadium.

The attached document above must be filled out, completed, and returned to Kirk Moats in Alumni Coliseum at least one week prior to the tryout.
 
All participants must bring proof of insurance and must be registered with the NCAA Eligibility Center.  Participants must also have proof of having had a physical within the last six months and must turn in proof one week prior to the tryout.  An assumption of risk form can be signed on the day of the tryout.
